Approval Form Online at Midwest! Did you know that the form needed to get your hazardous or non-hazardous waste stream approved from shipment into one of Midwest’s facilities is available in PDF format on our website? The Waste Profile form is available on the main Waste Disposal page and all subsequent pages in that section. The first step to disposing of your waste is completing our Waste Profile Form and submitting it to our Approvals Department. The form is then reviewed for completeness, technical accuracy and compared to our laboratory analysis if applicable. Your Sales Representative or Approvals personnel will contact you for additional information if necessary. Once final review is completed, a Waste Approval Code is assigned to the waste stream and a quotation is generated. After these steps are completed a waste stream can now be scheduled for pickup.
